Guidelines and Conditions of Entry - Return to work Awards 2012

The aim of the awards is to recognise and celebrate the
outstanding contributions made by all stakeholders who play a
pivotal role in the rehabilitation and return to work of injured
workers.
Nominations for the Return to work Awards are welcomed from
Monday 5th March 2012 and will be accepted until midnight on 8th
June 2012. Nominees will have until midnight on 15 June 2012 to
complete their nomination.
You may nominate yourself or someone else for an award. The
person who is being nominated must complete the required nomination
form. The Awards team will contact you to offer assistance to
complete your nomination, however should you wish to contact us,
please call 1300 389 177 or email expo@qcomp.com.au.
Once you have completed the nomination please submit by hitting
the 'submit your nomination now' button at the bottom of the
nomination form. You will then receive confirmation of the receipt
of this nomination.
Completed nominations will undergo two rounds of judging in July
and finalists will be judged in September 2012. The judging
panels will consist of independent representatives from business,
industry, unions, the public sector and Q-COMP. For further

Winners will be announced at the award ceremony on Tuesday 30th
October 2012.
The award categories for the Return to work Awards are as
follows:
Category One - Case Management Achievement Award
This category is open to all injury or case managers working
within the Queensland workers' compensation scheme. The award
recognises an individual's successful delivery of injury and/or
case management within the past 12 months.
To be eligible you must:
- be an injury or case manager working in Queensland within the
Queensland workers' compensation scheme
- have contributed to rehabilitation and return to work of
injured workers over the past 12 months.
The winner of this award will have consistently demonstrated one
or more of the following:
- a strong commitment to the rehabilitation and stay at/return to
work of injured workers;
- innovation in case management and early intervention strategies
ensuring injured workers stay at/return to work in an efficient and
effective manner;
- actions above and beyond the standard practice;
- effective communication and relationships with key
stakeholders
- effective contribution to the return to work culture in
Queensland.
Category Two - Rehabilitation and Return to Work
Coordinator Achievement Award
This award recognises an individual's outstanding efforts in
rehabilitation and return to work of injured workers within the
past 12 months.
To be eligible you must:
- be registered with Q-COMP as a rehabilitation and return to
work coordinator;
- have contributed to rehabilitation and return to work of
injured workers in Queensland over the past 12 months.
The winner of these awards will have consistently demonstrated one
or more of the following:
- a strong commitment to the rehabilitation and stay at/return to
work of injured workers
- innovation in the workplace and early intervention strategies
to have injured workers stay at/get back to work in an efficient
and effective manner
- actions above and beyond standard practice
- effective communication and relationships with key
stakeholders
- effective contribution to the return to work culture in
Queensland.
Category Three - Employer Achievement Award for Rehabilitation
and Return to Work
These awards recognise an employer's outstanding efforts in
rehabilitation and return to work of injured workers within the
past 12 months.
To be eligible you must either be a:
- small to medium employer who is insured with WorkCover
Queensland or a self-insurer in the Queensland workers'
compensation scheme with less than 1000 employees
OR
- large employer who is insured with WorkCover Queensland or a
self-insurer in the Queensland workers' compensation scheme with
1000 or more employees;
and
- have contributed to the rehabilitation and stay at/return to
work of injured workers over the past 12 months.
The winners of these awards will have demonstrated one or more of
the following:
- a strong commitment to the rehabilitation and stay at/return to
work of injured workers
- innovation in the workplace to have injured workers stay
at/come back to work in an efficient and effective manner;
- actions above and beyond standard practice;
- how their organisation aligns its strategic objectives to
ensure better return to work outcomes;
- effective strategies to improve the stay at/return to work
culture in their workplace/industry.
Category Four - Injured Worker Achievement Award (Serious
Injury, Return to Work, and New Career)
These awards recognise an individual's successful rehabilitation
and return to work within the past 12 months following a
work-related injury.
To be eligible you must have had:
- a work-related injury compensated under the Queensland workers'
compensation scheme;
- a successful and sustainable return to work within the past 12
months.
The winners of these awards will have demonstrated one or more of
the following:
- a demonstrated strong commitment and determination to succeed
in rehabilitation and stay at/return to work;
- achieved a sustainable stay at/return to work with an
employer;
- able to identify the positive effects of rehabilitation and
stay at/return to work and show how they have been an inspiration
to others around them.
Category Five - Health Provider Achievement Award for
Rehabilitation and Return to Work
These awards recognise health and rehabilitation providers' best
practice services in returning injured workers to the
workforce.
To be eligible you must be:
- an individual who has provided treatment and/or recovery and
stay at/return to work services for injured workers in the
Queensland workers' compensation scheme over the past 12 months -
including individuals practicing in medical, allied health and
vocational rehabilitation.
OR
- an organisation that has provided treatment and/or recovery and
return to work services for injured workers in the Queensland
workers' compensation scheme over the past 12 months - including
organisations involved in medical, allied health and vocational
rehabilitation.
The winners of these awards will have consistently demonstrated
one or more of the following:
- proven evidence of improved health, recovery and stay at/return
to work outcomes for injured workers;
- demonstrated excellence in service delivery (individual) and a
multidisciplinary approach to service delivery (organisation);
- a new approach to implementing recovery and return to work
strategies;
- effective strategies to improve the return to work culture in
Queensland.
Conditions of Entry -
Return to work Awards 2011
- Entries must be received by 5pm Friday 8 June 2012. Late
nominations will not be accepted.
- To be considered complete and to be accepted, entries must be
completed on an official nomination form and all relevant data
uploaded.
- The judges' decision is final. Judges reserve the right not to
allocate an award in any category if in their opinion there are no
entries of sufficient merit.
- Submissions will not be returned to entrants.
- Entrants may be reasonably required to be available for
presentations and media interviews with respect to their award
entry and will take all reasonable steps to make themselves
available for and participate in such presentations/interviews.
Entrants also consent to the reasonable use of information
contained within or related to this nomination for promotional and
publicity purposes.
- Entrants must disclose any of the following (to the extent
relevant to the particular entrant), (Q-COMP will perform checks to
verify this information):
a. WorkCover Queensland premium payments must be up-to-date;
b. any prosecution by WorkPlace Health and Safety Queensland under
the Workplace Health and Safety Act 1995 in the past two years
(1/06/10 - 31/05/12);
c. any prosecution by the Department of Mines under the Mining and
Quarrying Safety and Health Act 1999 in the past two years (1/06/10
- 31/05/12);
d. any fatal workplace accidents in the past two years (1/06/10 -
31/05/12);
e. any breaches of relevant industry standards or legislation, or
any criminal prosecutions or convictions in the past two years
(1/06/10 - 31/05/12);
f. any determination by Q-COMP, Self-Insurer or WorkCover
Queensland regarding non-compliance by the entrant with the
requirements of a rehabilitation program or return to work
plan;
g. substantiated complaints within the past 12 months (1/06/11 -
31/05/12);
- Q-COMP reserves the right to decline or withdraw an entry where
the entry or related person/s or organisations breach any of the
above conditions or do not meet entry criteria. Entrants will be
notified of such decisions;
- Q-COMP and/or judges must be allowed to visit the
workplace/location and/or contact appropriate endorsers to verify
the entry where necessary;
- Q-COMP will ensure that there is no disclosure to any third
party of any information provided by an entrant and marked
'confidential', except to members of the judging panel, without the
prior consent of that entrant or where such a disclosure is
authorised or required by law;
- Any personal information contained within or related to the
entries will be used by Q-COMP for the purpose of conducting the
Q-COMP Return to Work Awards 2012. By entering the awards, all
entrants agree that Q-COMP can use this information in any media
for future promotional and marketing purposes. Q-COMP will not use
the personal information of entrants for any other purpose without
prior consent of the entrant, or unless authorised or required to
do so by law;
- Employees of Q-COMP are ineligible to enter;
- An entrant that breaches any of these Conditions of Entry is
subject to disqualification.
